Below you will find our marital and single worksheets, specially designed to gather the relevant information for your personalized estate plan.

We can either fill the worksheet out together at our initial meeting or, if you wish, you can fill it out to the best of your ability and bring it with you at our first meeting. If you can’t answer a question, leave it for our meeting. We will be able to discuss all of your questions and issues with our shared goal of formulating an estate plan that fits you and your family’s needs.
